Prerequisite: Must have beaten Prime Tubal in a New Game Plus run, in Pishion Dry Lake. Location: In the Kerguelen Plateau head back to the lake with the rocks you jumped on to get the Aether's Wings heirloom and the rock in the middle will now have the scar. Prerequisite: You must have defeated ONLY Prime Tubal in Pishon Dry Lake before you beat any other bosses. Location: Head to the Nibiru Deep Room in Pishon Dry Lake, then jump towards the middle of the lake and destroy the left side of the wall under the bridge, and head into the hidden room where the scar will be. Prerequisite: Must have beaten Prime Irad in a New Game Plus run, in Sun Tower. Climb to the top of this area and enter the room with Lamech's memory and the scar will be there. Blow a hole in the left wall and jump into the area and up onto a hidden platform and enter the room.
Location: Go to Citadel Agartha and find the area with the hanging orange lanterns. Prerequisite: Must have beaten Prime Enoch in a New Game Plus run, in Stygian Study. Run as far to the right on the platform as you can and interact with a hidden switch to generate a bridge through the clouds, and hug the right wall that is below Enoch's memory, and dropdown to find a hidden room with the scar. Location: You will come to an area with nothing but purple clouds. Prerequisite: Must have beaten Prime Naamah in a New Game Plus run, in Kerguelen Plateau.
Location: In the area where you find Naamah's memory that grants you extra damage against her, halfway through the secret passage behind the rock will be a secret door (no pop-up will appear), interact with it to find the scar.
